Registering with the practice
To register with the practice, please complete the online registration form.
You do not need proof of address or immigration status, ID or an NHS number. Our new online registration service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper registration forms are available at the practice if you need one.
City Health Walk-in Service
You can access the City Health Walk-in Service while your registration is being processed.
If you require urgent but not life-threatening medical assistance that cannot wait until you are registered, please call 111. Following a telephone assessment by NHS 111 you will be provided with information about the appropriate service for your health problem. If you are unsure which health care service you need to visit, NHS 111 will signpost you to the most appropriate care for your condition - GP, local pharmacy, emergency department or an emergency ambulance response.
